NEOOC: Registration/Fees

Cuyahoga Valley Challenge (NA Champs)

To register for the event, use the standard standard USOF registration form, and mail it to:

Filling out the form:

Fill out number 1-5,7,8 as shown on form.
For #6, nothing is required.
For #9, if only participating one day, please indicate which day
For #9, please indicate how many children will be enrolled in baby-sitting (more info)

Fees:

19 years old and older: $20 single day, $40 for both days
Under 19: $13 single day, $26 for both days
USOF/COF/IOF discount: standard $3 per day (under 19 $1.50 per day)
Special Events: Separate cost, not included in registration (ice cream social)
E-card rental: $2 per day
Late fee:
- $10 ($5 if only participating one day) if post marked after 4/30/2004
- $20 ($10 if only participating one day) if post marked after 5/8/2004
Dinners: $12 ($8 for 12 years old and younger) - Includes Ice Cream Social
T-Shirts:Separate cost, not included in registration
Preview maps: N/A
Other: Host Lodging - indicate how many people and nights ($20 per person for single night, $35 per person for two nights)

Cancellation/Refunds:

Full refund if you cancel before 5/8/2004, %75 refund after 5/8/2004.
